Please plan to take care of co-payments, co-insurance and any outstanding balances at the time of service. We accept debit cards, Visa and MasterCard.
If we are not a participating provider with your insurance plan, or your plan does not cover our services, you will be responsible for any charges incurred. We will supply a receipt for your insurance company.
Always notify our office of any change in name, address, phone or insurance information.
We accept all major insurance plans. Our staff makes every effort to simplify billing and insurance claim procedures. Please bring your insurance card with you to every visit so that we may ensure that our records are accurate. If you do not have insurance coverage, full payment is required at the time of service.
To maintain continuity of care, will be happy to assist you with a referral to a specialist within the Trident Health System, upon the recommendation of your provider.
