Appointments
To schedule an appointment call (843) 795-0300. New patients are asked to arrive 20 minutes early to complete paperwork. You may download and print out the forms and complete paperwork here. Please bring the forms and your insurance card(s) with you to your appointment. Emergencies will be handled accordingly should they arise.
Cancellations
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments.
Prescriptions
All prescription refills will be done within 24 hours of notice to our office.
Release of Records
We will try to get a copy of your records to you at the time you ask; however in the event we cannot you will be able to get your records within 24 hours. You will have to sign a release for all records
